HCFA-1500 and CMS-1500 are the same form

If a payer, EHR, or office still says “HCFA-1500,” they mean today’s CMS-1500 (version 02/12).

The short answer

HCFA-1500 and CMS-1500 are the same claim form family. The Health Care Financing Administration was renamed the Centers for Medicare & Medicaid Services in 2001, and the form’s common name followed. If a payer asks for a HCFA-1500, send a CMS-1500 version 02/12.

Why the old name persists

Practice-management software, older payer manuals, and office habits still say “HCFA.” Search volume for HCFA-oriented queries remains high. The form you actually complete is maintained by the National Uniform Claim Committee (NUCC) as the 1500 Claim Form, version 02/12.

Do not look for a separate current “HCFA-only” form. Check the version number (02/12) and the NUCC instructions instead.

Common questions

Can I still submit a form labeled HCFA-1500?
Submit the current CMS-1500 version 02/12. The HCFA name is historical. Use official OCR-red stock or your payer’s electronic channel.
Did the boxes change when the name changed?
The form has been revised over the years (notably to 08/05 and then 02/12), but the rename from HCFA to CMS was a government agency rename, not by itself a new box layout. Use current 02/12 instructions.
