Where to get a blank CMS-1500 form
If you need official claim stock, get OCR-red forms from a legitimate supplier. A downloaded black-and-white PDF is usually not submittable.

The red-ink (and scale) constraint
CMS states that the only acceptable claim forms are those printed in Flint OCR Red, J6983 (or exact match) ink. A downloaded copy cannot be used for submission of claims, because your copy may not accurately replicate the scale and OCR color of the form. Photocopies cannot be scanned and therefore are not accepted by all carriers and DMERCs.
That is CMS’s own guidance — not a vendor upsell. Scale matters as much as colour. A PDF that prints at 96% is as dead as one printed in black.
Where to actually get one
CMS does not supply the form to providers for claim submission. CMS directs you to:
- the U.S. Government Publishing Office (1-866-512-1800),
- local printing companies,
- office supply stores.
Buy official 02/12 stock printed in the required OCR red. This site does not host a PDF copy of the form.
What a downloaded PDF is good for
A black-and-white or on-screen copy is still useful for:
- checking your work before you type onto official stock,
- keeping a file copy for your records,
- income-tax or complementary-insurance purposes (where a photocopy is acceptable).
It is not a submittable Medicare claim form when the contractor requires OCR-red originals.

After you complete the form on official paper (or in your billing system), use the checker to catch formatting defects before the claim comes back unprocessable.
